Abstract

Development of a potentially widely applicable ‘tool’ to address sometimes neglected ‘organisational and cultural’ precursors to major safety-related ‘events’ is described. Twelve ‘events’ across several industries were studied, and detailed ‘expectations of good practice’ developed. Associated ‘penetrating diagnostic questions’ were also then developed to enable the identification of potential operational vulnerabilities, and a ‘systems approach’ is outlined (which also addresses behavioural factors) to facilitate the design of effective interventions.
